Hard Copy Claim Form

If you are unable to file your claim online, you can choose one of the following options to receive and complete the claim form:

Download the PDF-fillable version of the claim form. 

The PDF version is a fillable form you can complete using your computer.  You can save an electronic copy of the form with your answers.  Once you have answered all the required questions, print and mail the form to the VCF at the address noted on the form. 

Print the PDF version of the claim form and complete it by hand. 

Print a hard copy of the form, write in your answers, and then mail it to the VCF at the address noted on the claim form.  You should keep a copy of the completed form.

Request that a hard copy form be mailed to you.

If you cannot print the claim form from the VCF website, call our toll-free Helpline at 1-855-885-1555.  The Helpline will mail you a copy of the form.
