The VCF has just posted two new Presence Statement forms to the website. These forms should be used for those claims where affidavits are being submitted in support of presence. Both forms include detailed instructions on how to complete the forms, and how to submit them to the VCF once they are completed.
- Witness Presence Statement – The Witness Presence Statement should be used as the template for all affidavits moving forward. If you received a Missing Information letter requesting additional affidavits, or more detailed ones, please submit the new or revised affidavit using the Witness Presence Statement. You should not proactively re-submit affidavits that have already been submitted. The VCF will contact you if additional information is needed.
- Victim Presence Statement – The Victim Presence Statement allows a victim to provide additional details regarding their presence. This form is optional, but may be helpful if a claimant has additional information about his or her presence that is not already provided as part of the claim form responses or in the supporting documents.
